NOTICE: I, the legal guardian do hereby give my permission for my child registered to participate in the Auditions and Gospel Show at The Belmont Luxury Barn. I understand that I am signing my child up to participate in a program that can involve physcial activity. I understand that there could be risks of physical injury from participating in these activities. I release the Belmont Luxury, it volunteers, and agents from liability for any injury from negligence. I agree to make all arrangements for my child's transportation to and from each event. I understand that I am required to remain at the facilities during each event. I understand that Belmont Luxury, Staff, Coordinators, and any other Belmont Luxury Volunteers are not responsible for my child. I agree to provide contact information & will notify Belmont Luxury if any information changes. I understand that failure to follow the Belmont Luxury Barn Rules and Regulations will be grounds for permanent dismissal from the premises and events.
